handle parameterized associations

This patch is an alternate approach to creating/updating associated models via parameter hashes submitted from forms. See ticket for background:

This patch is largely extracted from a working application, taking some inspiration from Ryan Bates' advanced recipe too. Fellow pivot Ryan Dy gets half the credit too.
Stay tuned for another patch that will add support for parameterized associations to the form_for helper.

I imagine this is no longer applicable, with the new multimodel forms stuff.
