Add proper :index support to form helpers

Commit 0da2aa6 (née [8786]) added incomplete support for the :index option to form_for/fields_for; this patch completes it by covering all of the helpers from ActionView::Helpers::FormBuilder, not just those defined in form_helper.rb.
