Merge Dispatcher and Middleware in Application object

action_controller/dispatch/dispatcher.rb and action_controller/dispatch/middlewares.rb configs are related to the "Application object".
We may want to magically point (w/ a deprecation) ActionController::Dispatcher some how to the default application object instance so old versions of config.ru and passenger don't totally break.
