Detect the existence of the test schema and autoload it if necessary

It's been a long time coming, but here is my patch. It removes the aaa_create_tables_test.rb file in favour of checking for the "accounts" table in helper.rb. If that table is missing, the schema will be auto-loaded.

    This doesn't quite do the same thing.

    If we add a new file to the schema, but don't drop the accounts table, the schema won't be updated.
