Lazy generation of ActiveModel::Name attributes

I've created a patch for the ActiveModel::Name class. Instead of calculate all the attributes on instantiation of the class, attributes are generated and remembered when used.
I've also added some basic documentation for the methods, plus completed the test coverage of the class by adding tests for the human and cache_key methods.
