[PATCH] Application object should instantiate non-vendored plugins

To ensure that plugin initializers are run with the correct binding, non-vendored plugins should be instantiated by the Rails::Application object (Plugin::Vendored.all already instantiates vendored plugins).

Although there is a comment on this method indicating it will be refactored, I didn't want to let this issue slip through the cracks.
