Rename Rails::Subscriber to Rails::LogSubscriber

After some discussion with the core team, seems like Rails::Subscriber name wasn't so clear on its purpose. This patch renames all of the references to Rails::Subscriber and subscriber method to Rails::LogSubscriber and log_subscriber respectively
