    I agree with Geoff.

    The problem is that we're not calling #lookup_by_extension ourselves. If we were, then we could .downcase before passing into the method. ...

    +1 on this. Attached is a patch that passes all current actionpack tests, with an extra test to verify that #lookup_by_extension is case-insensitive.

    I couldn't reproduce this either. I did, however, find that dirty tracking doesn't work with in-place manipulation of said serialized column:
