Respect consider_all_requests_local

The ShowExceptions middleware fails to respect the consider_all_requests_local setting. When developing locally I want to have the option to examine the way ShowExceptions handles rescue_action_in_public.
The generated environment defaults are the expected behavior for Rails apps; by respecting consider_all_requests_local, the behavior is completely determined by the environment configuration.
